PAY AND BENEFITS

Annual Pay Range: $68,868.87 - $87,447.48

Hourly Pay Range: $33.110035 - $42.042059

Salary offers will be made within the posted pay range and will be based on a candidate's experience (paid or unpaid) that is directly relevant to the position.

The Behavioral Health Division provides licensure supervision to all regular status employees who have completed a Graduate degree in a behavioral science-related field (social work, counseling, clinical psychology, etc.), and is actively pursuing licensure in that field.

There may be options for loan forgiveness while working with a government/public agency with the Public Loan Forgiveness (PSLF) program.

JOB DETAILS AND QUALIFICATIONS

Clackamas County's Behavioral Health Division is seeking a motivated and self-directed Forensic-Focused Mental Health Services Coordinator (ENCC) to join the Choice Model Program Team. This position focuses on providing care coordination for individuals with forensic or justice system involvement who meet the Choice Model criteria. The ENCC works closely with multiple systems to support clients in remaining in the most integrated, community-based settings aligned with their preferences. This includes collaboration with a wide range of partners, such as family, peers, natural supports, community mental health providers, hospitals, residential programs, and addiction recovery services.

The ideal candidate will have extensive experience working with adults involved in the forensic or justice system who experience severe mental illness, along with a strong understanding of mental health and residential systems of care. Success in this role requires expertise in person-centered, strengths-based, recovery-oriented approaches, as well as proven experience in care coordination and collaborating with diverse partners. Above all, this position is rooted in a commitment to empowering individuals to define and achieve their own recovery goals.

Required Minimum Qualifications/ Transferrable Skills:*

• Minimum of three (3) years relevant experience that demonstrates the required skills and knowledge to perform the duties of this position.
• Qualified Mental Health Associate (QMHA) as established by the Mental Health and Addiction Counseling Board of Oregon (MHACBO)

A Qualified Mental Health Associate (QMHA) means a person delivering services under the direct supervision of a Qualified Mental Health Profession (QMHP) and meeting the following minimum qualifications as authorized by the Local Mental Health Authority (LMHA) or designee:

(a) Bachelor's degree in a behavioral sciences field; or

(b) A combination of at least three years relevant work, education, training or experience.

Additionally, QMHAs must demonstrate the ability to communicate effectively, understand mental health assessment, treatment and service terminology and apply each of these concepts, implement skills development strategies, and identify, implement, and coordinate the services and supports identified in an ISSP.

• Experience working with individuals involved in the forensic/justice system, specifically the Aid and Assist program.
• Experience working directly with adults with Severe and Persistent Mental Illness (SPMI).
• Experience in care coordination and care management, including accessing and implementing service coordination plans and monitoring client progress toward goals.
• Experience with electronic medical records.

Preferred Special Qualifications/ Transferrable Skills:*

• Three (3) years of experience working with individuals involved in the forensic/justice system, specifically Mental Health Court and/or the Aid and Assist program.
• Experience and training in trauma-informed care.
• Qualified Mental Health Professional (QMHP) certification.
• Bilingual and/or bicultural skills.
• Familiarity with relevant Oregon Administrative Rules and statutes.
• Knowledge of Clackamas County services, providers, and resources.

TYPICAL TASKS
Forensic Population Support

• Provide specialized care coordination for the Aid and Assist/forensic population meeting SPMI (Severe and Persistent Mental Illness) criteria.
• Meet with members, their representatives, care providers, and related service providers in person, by phone, or using virtual technology.
• Conduct needs assessments and co-create choice care plans with members and care teams.

Care Coordination

• Communicate and coordinate with service providers to manage care for members engaged in CCBH's choice model team.
• Arrange and lead, or participate in, monthly multidisciplinary care team meetings using person-centered formats when possible.
• Ensure coordination and communication with the forensic system.

Documentation

• Document all actions and discussions within 24 business hours or as specified by internal processes.
• Participate in ongoing audits and monitoring of choice charts, identifying documentation improvements to meet audit criteria and passing scores.

Member Support and Advocacy

• Provide support, education, and guidance to help individuals achieve their goals.
• Identify and assist individuals in obtaining recommended and medically necessary services, as well as other resources needed to meet their goals.

Community Engagement

• Serve as a liaison with referral sources, community partners, service providers, and families regarding behavioral health services for Clackamas County residents.
• Provide technical assistance as needed and attend local, regional, and state meetings as assigned.

Administrative Duties

• Perform general office tasks such as copying, printing, faxing, emailing, mailing, and submitting requests for chart uploads.
• Track and manage communications and records requests.

Projects and Division Support

• Work on tasks, projects, and metrics to support behavioral health division goals and objectives.
• Participate in other duties as assigned to advance division initiatives.

WORK SCHEDULE

This position is included in the County's alternate workweek program, working 40 hours during a standard workweek of Monday through Thursday (Fridays off). Specific hours of work will be discussed with the candidate selected for this position at the time an offer of employment is extended. The exact amount of on-site/telework and schedule will be discussed with the hiring manager during time of offer. Telework locations cannot be located outside of Oregon or Washington.
